UOP Guidelines for Approving a BSHS Field Experience Agency

The following are key factors for approval of a placement:

1. Agency and the role you will perform must be human services oriented;

2. Your agency supervisor must have at least a B.A. degree in a human services field;

3. You must meet with your field supervisor at least once per week to receive guidance and training for your field work;

4. The field supervisor must agree to and sign the: • field agency agreement, • human services agency profile, • affiliation agreement, • weekly hour log, and • provide the UOP faculty with their evaluations of you.

5. The field agency must not be the same agency as your current employment unless your duties are significantly different from your regular job, in a different department, and with a different supervisor. (this can occur only once)

6. You are not to work alone with the agency's clients. Instead, a qualified staff should always be present.

7. Submit the Agency Profile, Affiliation Agreement (2), and Agency Agreement forms to your faculty or Denise Land (BSHS Lead Faculty) for approval.
